About Precise Home Care

Precise Home Care, LLC is a locally owned home health agency and is managed in Jacksonville, Florida. This agency is a Medicare certified, for-Profit LLC company which was organized on May 22, 2006 to provide patient care to patients located in Duval, Baker, Clay, Flagler, Nassau, St. Johns, and Volusia counties.

The agency’s first patients were seen March of 2007. Since then, the agency has grown as a direct result of its professional staff: the administrator, the clinical management staff, the community liaisons and clinical field staff.

Precise Home Care, LLC professional field staff includes Registered Nurses, Physical Therapist, Occupational Therapist, Speech Therapist, Medical Social Workers, and Home Health Aides. We continue to increase our field staff to meet our growing patient needs.

In our first month of operation, Precise Home Care, LLC admitted approximately 12 patients to our services. By the end of its first fiscal year in December of 2007 we serviced approximately 2,403 patients for the year.

To maintain Precise Home Care’s high professional standards, the agency utilizes the expertise of its professional management team, Dr. Derek Muehrcke our Medical Director, the Advisory Board and our highly trained and dedicated field staff.

Precise Home Care, LLC provides intermittent home care services seven (7) days a week, twenty-four (24) hours per day for Medicare, Private Pay, Private Insurances, VA and Indigent care patients.
